Teachers in the west work under the idea of locus parentis.
I wonder if this is the case in Japan?
I am sure that there is a degree of responsibility expected but in this case there doesn’t seem to have been any given.
Still, beach season is here and many parents will be taking their kids to the sea and just like in this pool, there will be drownings due to parents not recognising the danger of allowing their children to play without supervision.
The dangers of heatstroke are well reinforced in Japan but unfortunately death by water is not.

Your post is a nice summary of the state of affairs.
I would propose that Japanese society goes through a type of brainwashing with regard to rules. Who makes them and why they exist is very much secondary to the idea that they have to be followed even though commonsense blatantly dictates otherwise.
And as for lack of communication?

The number of infirm and elderly are rising yet businesses are failing?
So, what happens to the elderly not placed in a nursing home or receiving care at home?
It is hospitals that are receiving more and more elderly patients that are unable to look after themselves.
What happens to patients which receive basic care and are able to be discharged?
Their families are reluctant to accept them back home and care homes are full to the rafters.
There should be help for family members taking on the financial burden of caring for the elderly.
The government needs to lift its spending on the aged in Japan to take account of rising numbers.

Sometime today, the yen would have sunk to 162 to the dollar and the cost of a drink in a bar to a car tire will be on the rise for the ordinary Japanese person to rail against.
Meanwhile, Fumio Kishida’s planes (2) will have their aviation fuel ordered, on board provisions tallied and pilots’ rosters decided.
The reason is to attend a NATO meeting, of which Japan is not a member, halfway away, on the other side of the world.
What will be Japan’s contribution?
It is truly suspect.

If the NPA revised speed limits up to take account of more car safety features and reduced the number of traffic lights then most roads would be safer.
